Form issued by the Association of Old Fianna, to collect information for the Fianna Roll of Honour, to commemorate members of the Fianna who were killed in action or died as a direct result of their activities in the War of Independence. Issued by the Memorial Committee from an address at 52 Lower O’Connell Street, Dublin. Un-dated. Typed form, with details handwritten in blue ink. Form completed for a Mr John Healy of 188 Phibsboro Road, of C. Company, born 26 May 1901, died 27th April 1916 from a wound to the head. Occupation listed as a plumber’s helper. It is noted that there is a photograph and a hat and tunic of the deceased available. Signed by D. Stewart, Secretary. This is Sean Healy, the 15 year old member of Fianna Eireann who died as a result of a gunshot wound while couriering messages during the 1916 Rising.;
